Enrollment Forms

Please print, complete and return all of the following forms with the first tuition payment on July 1st.

Forms can be submitted via mail or scanned and email to MembershipPHCC@gmail.com. No student will be able to attend school before all paperwork is completed and returned to PHCC.  The child health form MUST be completed by your child’s pediatrician and returned before the first day of school.
