Below are all of the required forms for Perry Hall Children’s Center. Please print, complete and return all of the following forms with the first tuition payment on July 1st. Forms can be submitted via mail or scanned and email to [email protected]. No student will be able to attend school before all paperwork is completed and returned to PHCC.
Please note that your child cannot start in-person school until the Child Health Form is received.
